True story. About 3 or so years ago, a new company took over the management of the car parks at the hospital where I work. They introduced barriers and a pay on foot system. One day someone collapsed on the car park - they called us on the crash team to go out but we don't go outside as we're not insured and have no facility to bring the casualty into the building. Therefore the member of the public had to call an ambulance to a casualty on the hospital car park (mad in itself). When the ambo arrived, they went onto the car park, got the casualty on the vehicle to bring them to a/e, and the new car park manager would not let the ambulance off the car park (on blue lights) until the £2 parking charge was paid. The paramedic had to pay £2 out of his own pocket to get off the car park to bring the patient in. Ridiculous as that sounds, that's a perfectly true story.
