Hello and welcome to the LOC.

Given the notoriety of the Lexus diesel engines it could be a multitude of reasons but to begin with I would be looking first at the EGR valve which is prone to sticking due to carbon build up ( also given that his device only becomes operational once the engine has fully warmed up matching your symptoms).

There are plenty of DIY tutorials related to removing and cleaning the valve although new replacements are reasonably priced.

Another problem is the 5th injector involved in DPF regeneration which if faulty leads to over fuelling and white smoke.

Exhaust in a diesel must be really warm in order to be kept clean. Diesels are made for long hauls, not short tours. The engine in the 2.0tdi DSG Golf we had needed 5 - 10 minutes of high revolutions in order to pass emission test.
