My wife's just been in a bump, hit from behind... boot and bumper pushed in.  Wife is ok, so Police were happy for the car to be driven home

So, question is, do we use lexus assist, or go via insurance directly... worried about insurance using a cheap repair shop,  but not sure how it all works

I thought Lexus Assist was just breakdown cover but maybe I'm wrong.

Yes, does both, or it certainly used to.... they are supposed to arrange the repair via the insurance company, but guarantee genuine parts etc.... just wanted to know if anyone has experience of the service

I think that this service was discontinued several years ago.

just arrange repair via your insurers but tell them it is going to a Lexus bodyshop.

You may need to blag a loan car from repairer as insures wont cover it, but having said that TP insurance should cover it if your wife has been rear ended (excuse the expression).
