What's a Carbon clean? As in, getting carbon out of engine-or exhaust?

Usually a good 80 mph blast is recommended, especially if mainly town use is standard.

Here's me wondering if I should get another - for NON-town use!

I would not waste money on carbon clean. All you need to do is run your Lexus on 2-3 tanks of Shell  V power . It will also clean your injectors . Then every year  fill up the tank again and you will have a very clean engine. I have no involvement with Shell, but I have read much about it. BP have just launched a similar product. When I run my Lexus on it you can tell the difference, plus my wife's Honda's have run on it all the time and you can definitely tell the difference, plus the extra cost is minimised by better proven fuel consumption. Forget after market engine cleaners.

With todays fuels and oils and one of the best V8 engines ever built no need. More important would be oil and filter change more often than the usual schedule with lots of short journeys.
